Cytomorphological Spectrum of Thyroid Lesions Based on Bethesda Reporting System: Diagnostic Utility and Pitfalls

Introduction: Fine Needle Aspiration Cytology (FNAC) is the frontline diagnostic tool in assessing Thyroid enlargement. FNAC simple, easy to perform, cost effective procedure with rapid diagnosis. Other tests like Ultrasound, radionucleotide scanning, profile are adjuvant which more reliable. Aim: To categorise spectrum of lesions on according Bethesda system for Reporting Cytopathology (TBSRTC) and study its utility pitfalls. Materials Methods: This was a retrospective observational carried out Department Pathology at Government Medical College/Government General Hospital, Mahabubnagar, Telangana, India, from January 2019 December 2021. The analysis data done 2022 June 2022. fine needling performed cases enlarged gland, smears were prepared fixed 95% ethyl alcohol, stained Hematoxylin Eosin Papanicolaou stain. Cytohistopathological concordance wherever possible. Sensitivity, Specificity, Positive Predictive value, Negative value calculated. Results: Out 155 cytology cases, 8 (5.16%) unsatisfactory/nondiagnostic, 133 (85.80%) Benign lesions, follicular neoplasm/ suspicious neoplasm, No reported as Atypia undetermined significance, 3 (1.93%) malignancy malignant. studied 19 cases. One case Anaplastic carcinoma referred higher center further management. Conclusions: plays pivotal role diagnosis Swellings. reporting effectively categorises into various sub groups facilitates better communication between pathologist clinician resulting proper management patient. In this study, number predominated occurred females. Cases presenting nodular malignant advised surgical resection evaluation. found useful categorisation advising medical or management, urgency treatment extent surgery.
